A “Basic” crown is defined as a single crown made of porcelain fused to metal, having a metal “thumb” on the distal lingual for holding with hemostats, one shade from either the Vita or Bioform shade guides and filling the normal size and shape of the tooth it is replacing. Contacts will be present and occlusion will be light unless specified on the Rx.

 

All other custom design features will be billed out as time and skill level warrants. This includes multiple or custom shades, porcelain shoulders, fit to partial, metal collars, metal occlusions or linguals, unusual shapes or special requests made by the doctor or the patient that deviate from the “Basic” crown definition.

Pressable Glass restorations are not recommended for bruxers or anyone with a particularly heavy bite or unusual occlusal condition. Preps must be very rounded and not exhibiting any sharp angles. A shoulder prep is the most desirable prep design.

Photographic Services & Consult         

Patient Documentation

For our local offices and occasionally on special arrangement for out of town patients we offer our photographic services. Using the latest in a digital imaging system, we will come to your office and take digital photos of your patients for the purpose of recording their current oral condition as well as after the new restorations have been seated. We can do this in a spare operatory, in a consult room or even outdoors if a suitable area may be found. We try to be as least disruptive to your office as possible and still give you the needed images that may be suitable for framing or displaying in your office or a “Before and After” treatment book. A copy of a photographic release must be signed by the patient.

For offices outside the Atlanta Metro area, a nominal fee for driving time and mileage will be charged.

 

Website Photos

 

For your upcoming website development needs, Jon Berry has worked with a great website designer and a practice consultant to be able to provide a complete package for your website. Jon has a complete photographic studio with lights, backdrops and all of the equipment he will bring with him to make your photos look professional and appealing to your patients. We come to your office, evaluate the light in each room where you want to show off your daily patient environment or new equipment and then set up and shoot enough images to give you several “looks” at each scene. This service includes portrait photos of you and all of your staff as well as any patients you may wish to include in the shoot.

 

Rates are determined by the amount of time Jon will spend with you, including transportation time to and from your office and are consistent with his other photographic rates.

Hourly, 1/2 Day and Full Day Rates apply.

1/2 day rate is 3 to 5 hours

Full day rate is for any shoot lasting longer than 5 hours.

Any shoot lasting more than 8 hours incurs another day rate charge.
